Dental Insurance (PPO) & Claim Submission

If you have dental insurance, we will submit insurance forms/claims as a courtesy. Your plan’s coverage is an agreement between you and your insurance carrier, and benefits can vary by employer and policy.

Even when we submit claims, you are responsible for all fees charged by our office, including any portion not covered by insurance. If your statement does not reflect an insurance payment within 4–6 weeks, please contact our office so we can help confirm claim status.

Payment Responsibility & Monthly Arrangements

Any remaining balance after your insurance has processed is your responsibility. Prompt payment is appreciated.

If you need a monthly payment arrangement, it must be set up prior to the actual procedure. Financing availability and terms depend on the option chosen and (if applicable) credit approval.

48-Hour Cancellation & No-Show Policy

We understand unexpected events can interfere with appointments. If you are unable to keep an appointment, we ask that you provide more than 48 hours (2 business days) notice.

This notice allows us to offer the time to another patient who may need care, and it helps our practice manage reserved doctor and staff time. A fee may be charged for missed appointments or cancellations made with insufficient notice.
